Summary
Records the multi-document model selected in #633 as a new decision,
Open documents as tabs within windows: tabs inside independent windows, all in one process, with one pinned folder context per window and each saved file open in at most one tab across all windows. The rationale keeps the two measurements that decided it. Another tab costs a few MiB where another window costs tens, and separate processes each overwrite the others' settings and recent items when they exit. The consequences name what every delivering issue has to keep: opening never replaces a document, watchers and close requests belong to one window, closing asks once per window, and nothing is restored after a restart.
The Specification, Architecture, Reference, and Help describe delivered behavior, so they change with the issues under #639 rather than here.
