fix(op-reth): handle traces under failed parents

Summary

• mark successful descendants below a failed call as error traces with parent call failed
• preserve errors produced by the current EVM frame
• assign correct trace addresses to synthetic SELFDESTRUCT traces
• keep reverted event indices at zero
• add focused regression coverage for failed parents, SELFDESTRUCT paths, and error-event indexing

Context

This reuses the implementation from #13 for the active OP rollout. The behavior follows Chaintable/reth-x#30 and aligns OP-Reth output semantics with the Mantle pipeline.

Tests

• Not run locally by agreement; PR CI is the source of build and test validation.
• git diff --check origin/main...HEAD passed.
• Both cherry-picked patches have the same stable patch IDs as the original fix(op-reth): handle traces under failed parents #13 commits.
